As Vincent Pitisci describes it in his book, Genius of the Tarot, the central or focus card provides the key concept for the question, several other cards placed around it act as a "satellite" array of secondary considerations, and each of those has tertiary cards to expand further on the answer. His design (which he envisions as a spread) is far too iterative for me, even encroaching on an "Etteilla" level of complexity. The model that I think works best, at least in theory, is the Lenormand Grand Tableau.
